Our client requires a Full Stack Developer with good public sector Healthcare experience to join their project in enhancing their user experience and integration capabilities to support eReferral workflows directly within electronic medical record (EMR) environments. The work will enable clinicians to find and retrieve appropriate healthcare services and providers from PHSD while remaining within their EMR workflow.
The project will enhance SMART on FHIR integration to support EMR launch context, fhirContext, and standards-based FHIR read and write operations. It will also add support for the Ontario Context Management Services (CMS) standard using FHIRCast so that patient, user, encounter, and application context can be synchronized where required across participating applications.
The PHSD user interface will be modernized using React and TypeScript best practices, with a focus on responsive provider and service discovery, semantic search, intelligent search suggestions, filtering, and a streamlined eReferral experience. Advanced Google Maps capabilities will be incorporated to support geographic service discovery, including regional and polygon overlays, service clustering, and other map-based visualizations.
Backend Spring Boot microservices will be refined to apply user roles, permissions, and entitlements when searching across different PHSD and provincial datasets. The work also includes integration with NetIQ for identity and access management, IBM Master Data Management (MDM), FHIR-based Pub/Sub patterns, REST and FHIR APIs, OpenSearch, PostgreSQL, and supporting AWS cloud services to provide secure, scalable, and reliable interoperability.
Work is hybrid but need to be onsite on a needed basis at client location in Toronto, ON.

Mandatory Requirements
- Public Sector experience is a must have
- Strong hands-on experience developing modern React and TypeScript applications with SMART on FHIR applications embedded within EMRs, including SMART launch, launch context, fhirContext, and FHIR read and write operations and Experience implementing event-driven healthcare integration using FHIR Pub/Sub or FHIR Subscription patterns and supporting messaging services.
- Experience designing and developing FHIR R4/R4B and REST APIs supporting healthcare provider, service, organization, practitioner, location, and eReferral workflows.
- Experience implementing FHIRCast-based context synchronization or equivalent healthcare context management patterns, including support for Ontario CMS standards.
- Experience implementing advanced provider and service search using OpenSearch or Elasticsearch, including semantic search, relevance tuning, filtering, and intelligent search suggestions.
- Experience integrating applications with NetIQ using OAuth2, OpenID Connect, JWT, and role-based access control.
- Senior Full Stack development experience using Java, Spring Boot, React, TypeScript, REST/FHIR APIs, modern front-end engineering practices, and enterprise microservice architecture.
- Healthcare interoperability and embedded EMR experience including SMART on FHIR, fhirContext, FHIR read/write operations, FHIR R4/R4B, eReferral workflows, OAuth2/OIDC, and Ontario CMS/FHIRCast context management. 25 Points
- Search, mapping, security, and integration experience including OpenSearch semantic search and suggestions, advanced Google Maps capabilities, role/permission-aware services, NetIQ, IBM MDM, and FHIR Pub/Sub patterns.
- Comparable healthcare modernization experience, AWS and DevOps delivery, stakeholder collaboration, technical documentation, production support, and knowledge transfer.
Desirable Requirements
- Experience working with Ontario public sector healthcare organizations.
- Experience implementing or supporting Provincial Health Services Directory (PHSD), provider directory, healthcare service directory, or eReferral solutions.
- Experience developing SMART on FHIR applications that operate inside Epic, Oracle Health (Cerner), MEDITECH, or other Canadian EMR or HIS environments.
- Experience with Ontario eReferral workflows, central intake solutions, or integration with products such as RICOH, Novari, or comparable eReferral platforms.
- Experience using SMART on FHIR fhirContext and FHIR create, update, search, and transaction operations in embedded clinical applications.
- Experience implementing FHIRCast or healthcare context synchronization standards for patient, encounter, and user context.
- Experience integrating applications with NetIQ identity and access management services.
- Experience integrating provider or organization data with IBM MDM or other enterprise master data management platforms.
- Experience implementing semantic or relevance-based search, suggestions, autocomplete, and faceted discovery using OpenSearch or Elasticsearch.
- Experience with Google Maps Platform, geospatial search, polygon or region overlays, clustering, and map-driven search experiences.
- Experience with FHIR Subscriptions, FHIR Messaging, EventBridge, SNS, SQS, or related event-driven Pub/Sub integration patterns.
- Experience designing and operating cloud-native applications on AWS using ECS, Lambda, API Gateway, S3, CloudWatch, IAM, Secrets Manager, and related services.
